Martin M Brown MD FRCP Professor of Stroke Medicine Institute of Neurology, University College London and Consultant Neurologist at the National Hospital, Queen Square, London, UK Hugh Markus DM FRCP Professor of Neurology St George's University of London and Consultant Neurologist at St George's Hospital, London, UK Stephen Oppenheimer DM FRCP FACP Formerly Director, Cerebrovascular Program Associate Professor of Neurology and Medicine (Cardiology) The Johns Hopkins University School of Medicine Baltimore, Maryland and Professor of Neuroscience, NJ Neuroscience Institute John F Kennedy Medical Center, Edison, New Jersey, USA * Stroke causes over 4.3 million deaths worldwide per annum and is second only to ischaemic heart disease as the major cause of mortality throughout the world, according to the WHO Global Burden of Disease Study. However, the burden of the disease is much more than just mortality, because two thirds of patients survive their first stroke and half of these remain disabled. About one in six people will have a stroke at some time in their lives. Despite its importance, stroke has been neglected until recently. Several recent advances are transforming stroke management. For example, there are exciting advances in the drug treatment of stroke and in some units appropriate patients admitted within three hours of onset of ischaemic stroke are treated by thrombolysis. Patients with subarachnoid haemorrhage or intracerebral haemorrhage, particularly those with cerebellar haematoma, may require urgent neurosurgery or interventional radiology. These advances have been instrumental in the development of the new and exciting discipline of stroke medicine. Stroke medicine is a fascinating specialty, which provides constant interest in terms of diagnosis, while the frequency of death and disability caused by stroke remains a challenge to effective stroke management. Treatment and rehabilitation require that the underlying pathology, aetiology and risk factors have been identified, and an assessment made of the patient's impairment, functional disability, prognosis and social circumstances. Patients with stroke require active and enthusiastic management. For all types of stroke, there is good evidence that early admission to a stroke unit with a specialised team has significant benefits in terms of survival and reduction in long term disability. Stroke Medicine is a readable, informative text which describes this clinical approach to stroke, including succinct descriptions of the underlying pathophysiology of stroke and clearly expressed advice regarding clinical diagnosis and management. It will serve as an indispensable reference for medical students, MRCP trainees, SpRs, neurologists and stroke medicine consultants. ** Contents • Classification • Clinical evaluation • Imaging • Clinical anatomy • Epidemiology • Vascular dementia • Common causes of ischaemic stroke • Intracerebral haemorrhage • Subarachnoid haemorrhage • Dissection • Rare causes • Cerebral venous thrombosis • Neuroprotection • Acute medical therapy • Prevention • Rehabilitation
